A few Simple steps to make an appointment:
First and foremost you can call (206) 707-8494 anytime to make an appointment and get your questions answered. If you receive a voice mail message. We will return your call within 24 hours. If you want to take the necessary steps before your call:
We will need the medical records showing your qualified condition. (If you would prefer the option of our staff helping you obtain these records skip to the Contact Form).
- We will only need chart notes from your last 2 or 3 visits within the last year. Please do not send more than this.
- X-ray and MRI reports are not necessary but helpful. Report summaries only please. No films needed.
We will need to review your medical records before making an appointment:
- You will need to fill out our Request for Medical records authorization form or the form provided by your primary physician.
- Have your doctors office mail or fax your records to our office. If you have your records mail, fax or hand carry them to our office. Once we receive your records they will be reviewed. We will call you with the results and make an appointment.
Qualifying conditions: Use the Contact Form if you have questions about your particular medical condition.
- Cancer, human immunodeficiency virus (HIV), multiple sclerosis, epilepsy or other seizure disorder, or spasticity disorders;
- Intractable pain, limited for the purpose of this chapter to mean pain unrelieved by standard medical treatments and medications;
- Glaucoma, either acute or chronic, limited for the purpose of this chapter to mean increased intraocular pressure unrelieved by standard treatments and medications;
- Crohn’s disease with debilitating symptoms unrelieved by standard treatments or medications;
- Hepatitis C with debilitating nausea or intractable pain unrelieved by standard treatments or medications;
- Diseases, including anorexia, which result in nausea, vomiting, wasting, appetite loss, cramping, seizures, muscle spasms, or spasticity, when these symptoms are unrelieved by standard treatments or medications. Anyone may petition the Medical Quality Assurance Commission and the Board of Osteopathic Medicine and Surgery to add other terminal or debilitating conditions to the list.
